Teens need school forms for driver test over summer

As classes end for summer, the Georgia Department of Driver Services reminds students who need to test for an instructional permit or driver’s license over the vacation to get a DDS certificate of attendance form from their school before the end of the school year. Students planning for a road test will also need a Georgia Alcohol Drug Awareness Program certificate of completion.
A certificate of attendance must be signed and notarized from the student’s school and presented when applying for a permit or class D license. Typically, the form is valid for a month. But, the certificate of attendance forms issued in May 2009 will be accepted until Sept. 30.  
Applicants under the age of 18 not enrolled in school must provide a GED diploma, high school diploma or an attendance form from the student’s local board of education if in home-school.
The ADAP course is taught in both public and private schools, typically with the ninth-grade health curriculum. Makeup classes are also conducted and certificates must be obtained from the student’s instructor.
