The following are among the food service establishments inspected in March by the Liberty County Health Department. The department scores on a 100-percentile scale. Kitchens are inspected at least every four months and are required to post their score sheets in public. The score and selected comments from health inspectors are included in each report. March 1

• Kobe Japanese Steak House, 1050 E. Oglethorpe Hwy., Hinesville. Score: 91. Grade: A.

Remarks: (1) Observed employee go outside to discard of rice then return to the kitchen and started taking skin off thawing raw shrimp. I talked to the manager and employee about washing hands before working with food. New violation. (2) Observed cooked rice stored in the walk-in cooler not cooled to 41 F within 6 hours. Corrected on-site by discarding. (3) Need to add asterisk to menus for all undercooked or cooked to order food items to remind customers about consumer advisory. (4) Observed rice in a metal pan without a health element and facility does not have a time agreement with LCEH. Told the manager that facility needs to by a warmer for rice to stay at 135 or have a time agreement with LCEH. (5) Owner stated they keep their sauces for over 7 days. Need to email, fax, etc., recipe with ingredients for sauces to see if they are a TCS food.

March 2

• Subway #10624, 466 General Screven Way, Hinesville. Score: 100. Grade: A.

Remarks: (1) Need Certified Food Safety Manager within 60 days from receiving food service permit. (2) Need commercial hose for mop sink. Owner stated the he ordered the hoses and they will be delivered soon.

• Subway #27016, 771 Frank Cochran Rd., Hinesville. Score: 100. Grade: A.

Remarks: (1) Need to repair self-closing fixture on back door. (2) Need to replace broken base board behind employee door. (3) Need Certified Food Safety manager within 60 days from receiving food service permit. (4) Need commercial hose for mop sink. Owner stated the he ordered the hoses and they should be delivered soon.

• Subway #27017, 740 E. General Stewart Way, Hinesville. Score: 100. Grade: A.

Remarks: (1) Need Certified Food Safety Manager within 60 days from receiving food service permit. (2) Need commercial hose for mop sink. Owner stated that he ordered the hose and it will be delivered soon.

• Subway #47926, 2351 W. Highway 196, Hinesville. Score: 100. Grade: A.

Remarks: (1) Need to see exterminator agreement for facility before receive food service permit. Owner stated that he will bring the agreement to the office or email the agreement to me. (2) Need commercial grade hose for mop sink. Owner stated that he ordered the hoses and they will be delivered soon. (3) Need Certified Food Safety Manager with 60 days from receiving food service permit.

• Subway #5712, 531 W. Oglethorpe Hwy, Hinesville. Score: 100. Grade: A.

Remarks: (1) Need food/ commercial grade hose for mop sink. Owner stated that he ordered the hoses and they will be at the facility soon. (2) Need Certified Food Safety Manager 60 days from receiving food service permit.

March 3

• McDonald’s (Hwy 196), 801 E.G. Miles Pkwy., Hinesville. Score: 86. Grade: B.

Remarks: (1) Observed posted food safety manager certificates that were expired or people no longer employed at the establishment. Corrected on site by providing the updated certificates for current managers. New violation. (2) Observed build-up on soda nozzles in front soda machine. Corrected on-site by cleaning soda nozzles. Repeat violation. (3) Observed expired sliced apples in walk-in cooler. Expiration date 03/01/2016 Corrected on site by discarding sliced apples. New violation. (4) Observed food debris and food splatter on the interior of the microwave. New violation. (5) Observed McChicken Patty temped 129F was in warmer drawer that was set for hot holding breakfast foods. Told the manager to check warmer to make sure it is working properly and talk to the employees about checking equipment and temperatures. (6) Folded Egg temped at 128F was discarded during switch over from breakfast to lunch. (7) Shelled eggs kept out at room temperature near grill were discarded during switchover from breakfast to lunch. Talked to manager about employees keeping the shell eggs at room temperature more than allotted time frame. (8) Observed no paper towels in women’s restroom. Corrected on site by refilling paper towel dispenser. (9) Observed drain plugs missing from dumpsters. Corrected on site by replacing drain plugs.

March 4

• Waffle House Inc., 442 E.G. Miles Pkwy., Hinesville. Score: 96. Grade: A.

Remarks: (1) Observed warewasher not reaching the required 165 temperature to properly sanitize dishware. Corrective Action: Must use three compartment sink to wash rinse and sanitize. Warewasher may not be used until it can reach proper temperature. New violation. (2) Observed debris under shelves in dry storage area. (3) Observed water pooling under ice cabinet. (4) The diced tomatoes at 50 and 49F were prepped and placed on the prep line about an hour ago.

March 7

• Larry’s Giant Subs #1, 755 W. Oglethorpe Hwy., Ste 112, Hinesville. Score: 98. Grade: A.

Remarks: (1) Observed inspection sheet not posted for public view. New violation. (2) Observed facility without proper sanitizing strips. Facility needs to buy quat sanitizing strips for 3-compartment sink. New violation. (3) Observed/felt sticky material on cabinets under the soda dispensing machine. (4) Observed no date marker for commercially processed chicken salad. Corrected on-site by discarding.

March 15

• Captain Joe’s Seafood Restaurant, 317 S. Coastal Hwy., Midway. Score: 72. Grade: C.

Remarks: (1) Observed employee plate cooked fish with bare hands. Corrected on site by discarding fish. New violation. (2) Observed food debris on pans stacked on clean rack and on vegetable slicer. Corrected on site by washing, rinsing, and sanitizing equipment. New violation. (3) Observed items on salad bar and prep line above 41 degrees F. Corrected on site by discarding and rotating food items. The manager stated the food was prepped and put on the salad bar around 2:00. I told them to continue to cool down the food in the walk-in cooler and place food from the walk-in cooler on to the salad bar. The banana pudding was made this morning and had not cooled to 41 within 4 hours so I had the manager discard of the banana pudding. New violation. (4) Observed container of sanitizer and unlabeled spray bottle of degreaser stored on food prep table. Corrected on site by moving to shelf below table. New violation. (5) Observed wet stacking of pans on clean storage rack. New violation. (6) Observed that facility did not have sanitizer test strips for warewasher. New violation. (7) Observed moldy lemons and oranges in walk-in. Corrected on site by discarding. (8) Observed cracks in frame of slicer. Slicer should not be used. (9) Observed employee drinking out of bottle in kitchen area. Employees should drink from disposable cups with lids and straws in kitchen area. (10) Observed gasket on walk-in freezer door needs to be repaired.

• Captain Joe’s Seafood Restaurant (reinspection, March 21). Score: 80. Grade: B.

Remarks: (1) Observed moldy tomatoes stored in the walk-in cooler. Corrected on-site by discarding of moldy tomatoes. New violation. (2) Observed raw seafood (shrimp and fish) stored over ready-to-eat food (Alfredo and shrimp and grits sauce) in the prep line. Talked to manager about reorganizing cooler. The manager told the employee to change the food around. New violation. (3) Observed pipe leaking under the 3-compartment sink and handwashing sink. New violation. (4) Observed employee washing hands in the sink by the seafood cooler. If employees are going to use the sink to wash hands, the sink needs to be supplied with paper towels and soap. (5) Cook did not know proper cooking temperatures for cooked food items. Person in charge needs to train cooks on temperatures. (6) Observed dishwasher not meeting minimum sanitizing strength. Tested dishwasher 3 times. Do not use dishwasher until it is working properly. The facility can wash dishes in the 3-comaprtment sink. The dishwasher sanitizer started working by the end of the inspection. (7) Facility needs to add (*) on the menus for consumer advisory reminder. (8) Observed grease build up and black substance behind fryers and dishwashing machine. (9) Suggest letting the cooks have thermometers so they can take temperatures of cooked food items and facility use cold-temperature logs. (10) Observed employee food stored in the walk-in cooler with customers food. Need designated area for employee food stored away from customer food items.

• Frank Long Elementary School, 920 Long Frasier St., Hinesville. Score: 91. Grade: A.

Remarks: (1) Observed food stored in the red warmer and serving line below 135F. Corrected on-site by discarding of food and increasing temperature of the streamer by adding water. New violation. (2) Coleslaw was made today around 9:00. I talked to the manager about the new rule changes for cut leafy greens. Manager discarded of coleslaw on serving line and added ice around the containers of coleslaw to decrease the temperature to 41F or below. (3) Need to check red warmer and make sure it is keeping food 135F or above and check the seal to make sure it is keeping the hot air in the warmer. Do not use warmer if it is not working correctly.

• Lewis Frasier Middle School, 910 Long Frasier Rd., Hinesville. Score: 100. Grade: A.

Remarks: (1) Talked to manager about the new rules and discussed reportable foodborne illnesses. Provided handout about restriction and exclusion. (2) Manager placed chicken salad in the walk-in freezer to increase cooling time. The chicken salad was made 30 minutes before inspection. (3) Dishwasher not working at the time of inspection. Manager provided invoice from Stackpole Food Service.

March 16

• Button Gwinnett Elementary, 635 Taylor Rd., Hinesville. Score: 100. Grade: A.

Remarks: (1) Talked to manager about keeping cut leafy greens at 41F or below on serving line. (2) Discussed placing containers in ice and surrounding containers or time agreement. (3) The meatballs on the serving line below 135F were discarded. The second temperature was within range. Make sure employees are taking temperatures on the serving line. (4) Facility is not using dishwasher at the time of inspection because the dishwasher is leaking.

• Liberty County High School, 3216 E. Oglethorpe Hwy., Hinesville. Score: 99. Grade: A.

Remarks: (1) Observed no sanitizer test strips for 3-compartment sink. New violation. (2) Observed gasket not sealed (closing bottom of the warmer on the serving line). Observed no food in the warmer at the time of inspection. Manger placed do not use sign on warmer door and stated he has not been using warmer. Observed pizza oven at facility, Received specification on new equipment. (3) Observed burned out light in the walk-in freezer. (4) The pizza pans need to be washed, rinsed, and sanitized after every use unless facility obtains wax paper to line the pizza pans. If the facility obtain liners the pans need to be cleaned at the end of the day. (4) Manager moved cucumber salad to the walk-in cooler to continue to cool.

March 17

• Liberty Elementary School, 600 Edgewater Dr., Midway. Score: 91. Grade: A.

Remarks: (1) Observed food on the serving line below 135. Corrected on-site by manager discarding of food. Need to make sure all hot holding food is 135F or above. New violation. (2) Observed yogurt stored in the storage closet at room temperature. Corrected on-site by manager discarding of yogurt. The yogurt was left on the breakfast tray by one of the employees. (3) Observed black residue on the interior of the ice machine. Need to clean as soon as possible.

• Midway Middle School, 1951 Edgewater Dr., Midway. Score: 100. Grade: A.

Remarks: (1) The chicken nuggets were reheated before service to increase internal temperature to 165F. The chicken nuggets were sitting out at room temperature and the food temperature decreased. Initial cook temperature was 175F. (2) The ranch dressing and yogurt temperature was elevated because employees were portioning the items, then placed them in the walk-in cooler to reduce temperature.

March 18

• Liberty County Pre-Kindergarten Center, 206 Bradwell St., Hinesville. Score: 100. Grade: A.

Remarks: (1) Observed water leaking from the ceiling in the dry storage area. Need to repair leak as soon as possible. Will follow up on leak

Manager put in a work order to have the leak repaired. The leak is not over any food on single-use items.

• Lyman Hall Elementary, 1396 Shaw Rd., Hinesville. Score: 100. Grade: A.

Remarks: (1) Facility is not using dishwasher during inspection. (2) Talked to manager about taking temperatures on the serving line, during breaks to make sure hot holding food is 135F or above and cold holding food in 41F or below.

March 21

• Joseph Martin Elementary School, 315 Coates Rd., Hinesville. Score: 100. Grade: A.

Remarks: (1) Gave manager information on reportable foodborne illnesses. (2) Talked to manager about proper procedures for handling ready-to-eat food items. (3) Talked to manager about increasing the water temperature on the serving line to keep food at 135F or above. Manager adjusted thermostat on serving line. At the time of inspection all food was at 135F or above because food just came out of warmer.

• Snelson-Golden Middle School, 465 Coates Rd., Hinesville. Score: 89. Grade: B.

Remarks: (1) Observed milk not maintained at 41F or below in the milk cooler. Observed sliding door propped open which caused the milk temperature to become elevated. Need to keep the door closed as much as possible to make sure milk temperatures are not fluctuating during lunch service. Suggest placing a close door sign on the cooler or have employee periodically check the cooler door. Repeat violation. (2) Observed pizza in the warmer/steamer below 135F. Corrected on-site by reheating pizza and adding more water to steamer to increase temperature. (3) Observed baked on residue on the outside of pans. Need to scrub pans to make sure all residue is removed from the pans.

March 22

• Taylor’s Creek Elementary School, 378 Airport Rd., Hinesville. Score: 99. Grade: A.

Remarks: (1) Observed outside dumpster lid open when not in use. New violation. (2) Observed ServSafe certification not posted. Corrected on-site by posting certification. Facility has two serving lines, need to post original certification at one line. (3) Observed water damaged ceiling tiles above serving line. Need to cheek area for leaks and change tiles.

• Waldo Pafford Elementary, 2550 W. 15th St., Hinesville. Score: 100. Grade: A.

Remarks: (1) Talked to manager about cut leafy greens on the serving line.

March 23

• Alternative Learning Campus: Coastal Academy, 100 Deen St., Hinesville. Score: 100. Grade: A.

Remarks: (1) Did not observe employee delivering food at the time of inspection. Employee is using warming bags to deliver food to each class during lunch. The food stays at the school during breakfast.

• Alternative Learning Campus: Horizon, 212 School House Rd., Hinesville. Score: 91. Grade: A.

Remarks: (1) Observed hot hold (chicken fajitas, corn and beans) and cold holding food (milk) not delivered to the school at the proper temperatures of 135F and above and 41F or below. Person in charge did not serve the hot holding food that was 135F or below. New violation.

• Bradwell Institute, 100 Pafford Rd., Hinesville. Score: 99. Grade: A.

Remarks: (1) Observed current inspection report not posted for the public to view. The inspection that was posted was 03/26/2015. New violation. (2) Need to individually wrap ready-to-eat produce sent to alternative school since person in charge is not at both facilities. (3) Observed sanitizer water at 132F. Per manufacturer’s instructions, water needs to be at 65F-75F. Employee was still setting up the sanitizer sink and he is going to add cold water to the sink. (4) Pizza pans need to be washed, rinsed, and sanitized daily unless more information is provided about equipment.

• Youth Challenge Academy, Bldg. 13505, East 16th St., Fort Stewart. Score: 97. Grade: A.

Remarks: (1) Observed wet wiping clothes stored on prep table. Corrected on site by removing. New violation. (2) Observed floors not sealed properly and bulging up in the walk-in cooler and walk-in freezer. Floors in walk-in cooler and freezer need to be repaired. (3) Observed onions in storage bin starting to mold, corrected on site by discarding. (4) Observed no paper towels in dispenser at hand washing sink, corrected on site by replacing towels.

March 25

• Awesome Wings and Things, 1310 Shaw Rd., Hinesville. Score: 96. Grade: A.

Remarks: (1) Observed desserts stored in the front reach-in cooler past expiration date. Corrected on-site by discarding. Food made 03/18/216. Corrected on-site by discarding. New violation. (2) Observed baked potatoes stored at room temperature. Employee brought the food out of the refrigerator around 11:30. Corrected on-site by placing potatoes in the reach-in freezer to decrease temperature. (3) Observed grease residue on crates under grease containers. (4) Need written procedures food non-continuous cooking procedures (fried chicken).

• Izola’s Country Cafe, 809 Willowbrook Dr., Ste 106, Hinesville. Score: 90. Grade: A.

Remarks: (1) Observed okra stored on the serving line at room temperature below 135F. Corrected on-site by discarding. New violation. (2) Observed last inspection sheet not posted. Inspection that was posted was 82 which was conducted on 06/25/2016. New violation. (3) Observed burned out light bulb in the rear storage area. Need to change as soon as possible. (4) Need to make sure thermometers are properly calibrated every day. The cook’s thermometer was reading different temperatures than my thermometer. (5) Observed black substance on the interior of the interior of the ice machine.

March 28

• Baymont Inn & Suites (Breakfast), 773 Veterans Pkwy., Hinesville. Score: 91. Grade: A.

Remarks: (1) Observed milk on the breakfast buffet above 41 F. Corrected on-site by discarding of milk and replacing milk with milk stored in the refrigerator. New violation. (2) Observed cabinet in disrepair with cardboard covering the hole. (3) Observed ambient thermometer not reading proper temperature in the refrigerator. (4) Observed faucet leaking at 3-compartment sink. Person in charge stated that the maintenance man is going to repair faucet. (5) Observed burned out light bulbs in the rear storage area.

• Hot Wing Express, 794 E.G. Miles Pkwy., Hinesville. Score: 100. Grade: A.

Remarks: (1) Owner needs to discard of any remaining fried chicken stored on time at the end of the work day even if it is before the 4 hour time limit. Suggest limiting the amount of chicken the facility cooks at night. (2) Employee drink needs to be in disposable sups with lid and straw.

March 29

• Uncommon Grounds of Hinesville LLC, 809 Willowbrook Dr., Hinesville. Score: 100. Grade: A.

Remarks: (1) Observed rusty floor tiles in the rear facility by the by the walk-in cooler. Owner has 90 days to replace rusty floor tiles. (2) Need to repair rear hand washing sink when facility starts serving food. (3) Facility is just serving coffee.

March 4

• Wings-N-Things Funnel Cakes, 182 Wildernest Rd., Ludowici. Score: 96. Grade: A.

Remarks: (1) Observed grease accumulated on kitchen floor. New violation. (2) Outer openings of establishment cannot be properly sealed when not in operation. New violation.
