Diversity Health Center’s Healthy Outreach Prevention Education (HOPE) Project was awarded $50,000 March 29 from the Susan G. Komen Coastal Georgia to target the uninsured and under-insured residents of Liberty and Long counties with a focus on African-American women.

The project will provide breast health education, 255 screening mammograms, 40 diagnostic mammograms, 20 ultrasounds, two biopsies and 250 client navigator services.

Client navigator services include translation services, follow-up care, education, transportation and linkage to available county resources.

Educational outreach will be held in collaboration with churches and community and cultural group events that Diversity Health Center partners with throughout the year. These events will be utilized as a source of connecting with the targeted population.

Mobile mammogram services will be provided in rural areas of Long County to promote access to breast health care. Diversity Health Center will work in partnership with Liberty Regional Hospital to provide screening and diagnostic exams.

Grant-funded services will be provided through March 2019.

Diversity Health Center has three locations throughout Liberty and Long counties. Call 844-877-2227 to schedule an appointment or for more information.
