Ga. Blue Cross Blue Shield to offer house calls to retirees

ATLANTA — Contrary to popular belief, house calls are not a thing of the past. Blue Cross Blue Shield of Georgia, on behalf of State Health Benefit Plan, will offer house calls to all SHBP members enrolled in a Medicare Advantage plan with the help of a contracted vendor, Matrix Medical Network.
With the program, a visiting clinician will conduct — at no charge to the member — a comprehensive health evaluation, including basic health screenings, such as recording the member’s height and weight, body mass index and blood pressure, as well as reviewing the member’s medications.
SHBP MA members will receive a brochure and letter asking them to take part in the hour-long, in-home visit, which is conducted by a licensed clinician. The mailers will be followed by a phone call from Matrix Medical Network to set up an appointment.
Brochures also will be sent to the members’ primary care providers to make them aware of the program and its purpose of supporting the care services provided to SHBP MA members.
Matrix Medical Network will conduct the exams, featuring local nurse practitioners who are familiar with local health services, including social services and doctors. SHBP MA members are not required to participate but are encouraged to do so to head off any potential health issues or to simply establish a detailed baseline assessment.
To participate, MA members may call Matrix Medical Network at the number listed in the House Call Program brochure or the introduction letter.
Matrix Medical Network also will be reaching out to the SHBP MA members.
